Join us and receive free health screenings from the Florida Department of Health in Polk. They will be offering, the following tests/screenings:
– BMI
– Blood Pressure
– Blood Glucose
– Sexually Transmitted Infections (STI)
They will also provide health education and referrals to community partners.
Polk County staff will also be available to assist you in learning more about the Polk Healthcare Plan (PHP), determine eligibility and get you registered. The plan provides access for members to receive primary care, behavioral healthcare, specialty care, medications and more.

PHP is not health insurance, so if you have insurance or other health coverage you may not qualify.
PHP is a part of the county’s healthcare safety net services, funded by the half cent sales surtax for indigent healthcare.
