12 Valley restaurants fail health inspections in July 2019

PHOENIX — On the first Tuesday of every month, ABC15 takes action to share restaurant report cards from around the Valley. In July, 12 restaurants and food establishments failed to pass their health inspections with a “C” or better.

PHOENIX

Luke’s of Chicago
1602 E. Indian School Road, Phoenix AZ 85016

Violations: Observed employee working the front register handling money then putting on gloves to prepare ready to eat food. Employee was educated on proper handwashing techniques and advised to wash their hands prior to changing tasks and between glove changes; Observed employee wash soiled hands while lathering for a total time of 3 seconds. Employee was educated on proper hand-washing procedure; Observed container of lemons in the walk in that contained roughly 20% spoiled lemons. 10 of the lemons were moldy while 6 were shriveled and rotten.

Violations: During the inspection, observed server buss dirty plates into dishwashing area, then proceed to pick up order to for service without washing hands in between tasks; Also employee area was observed handling dirty dishes then clean dishes without washing hands and changing between task; At time of inspection, the hot water at faucet at 3 compartment sink could not be turned on or off except by use of valve under sink. Also the spray nozzle next to dish wash machine was not functioning properly with water continuously flowing from it.

La Canasta Mexican Food

Violations: At the dish washing area, observed an employee handling dirty dishes with gloves on and loading them into racks. The employee then proceeded to remove a clean rack of dishes from the dish machine, and began touching the clean plates with his gloved hands; At the hand sink in the chips and salsa area, observed an employee ring out and rinse a cleaning towel in the hand sink. Also at the hand sink nearest to the order taking area, observed a kitchen employee filling up a metal pan with water at the hand sink. Reminded both employees that hand sinks shall only be used for washing hands; In the main kitchen, observed a dicer with organic matter stuck in the blades and inside of the push part;

Violations: Meat slicer being stored as clean and not used since yesterday had large build-ups of food debris on blade; No hand drying towels available at hand washing sink across from walk-in cooler; Cooked cactus in walk-in cooler had the date mark of [7/8/19 – 7/13/19] and was voluntarily discarded during inspection.

EAST VALLEY

Village Inn

Violations: An audit of employee food handler certificates was completed in the establishment had 5 out of 26 employee Food Handler Certificates (second consecutive violations); Observed cooked chicken hot holding on top of grill in hot water in two stainless steel pans. Internal temperature of cooked chicken was 121*F; notified and verified by Person In Charge (PIC). PIC discarded improper hot holding cooked chicken; Observed one cookline refrigerator with ambient temperature 46.4*F verified with min/max thermometer. Another cookline refrigerator had ambient temperature of 44.7*F. Another cold holding refrigerator at front of establishment had ambient temperature at 44.9*F. Notified Person In Charge (PIC); PIC will put work order for cold holding equipment in the establishment.

Violations: Audit of employee food handler certificates in the establishment was conducted due to repeat violation…Audit resulted in 2 out of 33 employee food handler certificates; Observed employee hand washing for 5 seconds. Employee proceeded to put on gloves and handle food equipment. Discussed with employee that handwashing needs to be 15 seconds or more; Observed employee with gloves handling raw eggs in shell. Employee cracked raw eggs onto pan. Employee discarded gloves. Employee put on new gloves. Employee began plating another meal;

El Alamo Carniceria

Violations: Observed a dead German roach on its back lying on the floor behind the steam table where boxed beer is stored; Observed cooked pork skins with meat (chicharron de pella) in the hot case, per kitchen employees this item was left over from last night and was out on time control past its allowed 4 hours. Therefore item was discarded; Observed a wiping cloth bucket being used to wipe down cutting boards, the chlorine used to make sanitizing solution was a bottle of ‘AWESOME BLEACH with FRESH SCENT’;

Tempe Elks Lodge

Violations: Person in charge of the kitchen was not aware of correct date marking, calibration of food thermometer, reportable diseases. The PIC has a Certified Food Manager certificate that is expired; Half cut cabbage and bagged salad mix in walk in cooler found with black mold and slime. Bagged salad mix in reach in cooler also slimy. Container of beef broth in reach in cooler with floating mold; Case of raw shell eggs stored in shelf in walk in cooler above whole produce. PIC moved eggs to a lower shelf.

The Monastery at Falcon Field

Violations: Observed raw ground beef thawing in container in handwashing sink. Instructed employee to remove ground beef from handwashing sink. Sink was cleaned and sanitized at time of inspection; Observed raw beef patties being stored over lemons, bacon and Jalapenos in sandwich prep table. PIC re-arranged items at time of inspection and moved raw patties to bottom shelf. Raw proteins must be stored according to their cooking temperatures to prevent cross contamination.

Oregano’s Pizza Bistro

Violations: Observed the employee at the grill line work with raw meat and take their gloves off and put new gloves on without the benefit of washing their hands when changing tasks; Observed the can opener in the back of the kitchen with a large accumulation of organic matter on it; Observed the cooked pasta noodles in the cooler pull out drawer below the grill line in the kitchen with an internal temperature of 50 degrees F. The PIC stated that those noodles had been there since the night before. The person in charge discarded the noodles; Observed a large accumulation of grease and debris building up throughout the kitchen around the walls and corners near the three compartment sink and grill area.

Denny’s

Violations: Flat of raw shell eggs (some broken) stored directly over open bag of spinach and open container of cut tomatoes in reach in cooler directly next to cook line. PIC relocated eggs to proper location; Four pans of hashbrowns-49-54F in walk in cooler and cooler drawer unit under cookline. PIC stated that hash browns were made the previous day and placed into walk in cooler to cool. PIC discarded; Chemical dispenser attached below atmospheric vacuum breaker at mop sink..

Manuel’s Mexican Restaurant Tempe

Violations: Observed a towel under a cutting board being used to prevent it moving. Use a rubber mat instead for this to prevent potential contamination; Observed a covered pan of refried beans in the walk-in at 47 Deg. F. from the previous night per manager. Item was embargoed at the time of inspection. Reviewing of the required cooling log reviled that refried beans cooling production is not being logged consistently and had not been logged for the previous day. Repeated violation.

WEST VALLEY

No restaurants in the West Valley inspected during the month of July failed their health inspection.
